                Dear The Liberty Clinic patient, We appreciate that you’ve trusted our primary care providers to take care of your health over the past several years! We’re sad to notify you that we will longer be providing primary care services after January 31, 2024.

We understand that finding a new primary care physician is challenging. We’ve been working with Community Health Care providers and they are able to take on new patients at all of their locations. You can contact their Patient Navigators at (330) 854-2510. To view their locations, you can visit www.chci.com.

Whoever you choose as your new primary care provider, just complete and return the enclosed Medical Record Release form via mail to 4875 Higbee Ave NW, Canton, OH 44718, or fax to (855) 373-7053. We will forward copies of your records for you. You can also log in to your CareSpace portal (CareSpacePortal.com) to download and print your records as well. If you need assistance with the CareSpace portal, call us at (330) 933-4536.
